Through LiveWell 4your health. your life ., Nissan is investing new resources in the health and well being of our employees and their families. To start, we want to invite you to participate in a voluntary health screening.

The voluntary health screening is available to employees this year and is one of two easy steps you must take to earn the maximum Nissan contribution to your 2009 health plan account (Health Reimbursement Account or Health Savings Account). More importantly, completing the screening positions you to take advantage of valuable new wellness programs and support to improve your health and help manage chronic conditions. Health screening results are confidential and free for employees who visit a participating network provider.

To obtain your free health screening, follow the steps below: 1. Obtain an onsite health screening at a major Nissan location during August/September (Canton, Dallas, Decherd, Farmington Hills, Franklin and Smyrna). 2.

Decherd, Smyrna and miss the onsite screening conducted by StayWell in August/September, make an appointment at the onsite CHS Medical Clinic. 3. If you work at a location other than the ones listed in #2 above, follow these steps: a. Schedule an appointment with any BlueCross BlueShield (BlueCard) participating network physician. b. Bring this packet, including the enclosed letter from Blue Cross Blue Shield of Tennessee and the StayWell Health Care Provider Form with you to your BCBS physician. c. Complete the form by filling in your name, your employee ID and date of birth. The physician office will input your screening results on the form. Note that your total cholesterol, HDL and glucose counts may not be provided on the same day as your doctor 9s visit. In this case, you will need to wait for those results from your doctor 9s office in order to complete the StayWell Health Care Provider Form. d. Once you have received all screening information requested on the Health Care Provider Form, sign and return it to StayWell in the prepaid postage envelope included with this packet. Be sure to schedule your appointment with your BCBS physician as early as possible in order to return the StayWell Health Care Provider Form by December 1, 2008 . Your individual results are confidential and will not be shared with Nissan at any time. Nissan will only be provided aggregate data that summarizes the overall health and risk factors for employees as a whole.
